Welcome and thank you for selecting us for your medical care. This information sheet provides an overview of our office policies. If you have any questions, please do not hesitate to ask. We are here to help. OFFICE HOURS: Our office hours are: On Monday, Wednesday and Friday 7:00 a.m. – 4:00 p.m. and Tuesday and Thursday 9:00 a.m. – 5:00 p.m. We close from 12:00 to 1:00 p.m. daily for lunch. RECEPTION AREA: Seating in our reception area is limited. Please try to limit the number of people who accompany you to your appointment. It may be inappropriate to have small children in the examination room with you. If possible, please arrange for someone to supervise small children during your appointment. Please do not leave children unattended in the waiting room. TELEPHONE CALLS: To be fair to scheduled patients, every effort is made not to interrupt clinical staff during patient visits. Patient calls are triaged by administrative staff. Please provide information as requested of you so that we can appropriately respond to your question or concern. If necessary, arrangements can be made to return your call at a specified time but usually calls are returned after office visits are complete. We may be able get answers for you in between patient visits and will call you back as soon as possible. TELEPHONE CONSULTATIONS: There are times when a detailed telephone consultation with patients is appropriate. Please be aware that the office does charge for telephone consultations. This office does not bill insurance companies for telephone consultations. The fee for this service is $45. Prepayment of this fee is required prior to the telephone consultation. APPOINTMENTS: If it is necessary to reschedule or cancel your appointment, please call us by 12:00 noon on the day prior to your appointment. If you need to cancel a Monday appointment, please leave a message on the answering machine prior to noon on Sunday. If you do not cancel your appointment with this notice, you will incur a $30 cancellation fee. If you arrive more than 15 minutes late for your appointment time, we may have to reschedule your appointment. This will also be considered a late cancellation. We know your time is valuable and we make every effort to seet you at your scheduled time. However, there may be patient emergencies in the office or at the hospital that can cause schedule delays. We will always let you know if there is a delay. We appreciate your patience and will work with you in any way possible to ensure that you are seen at a time that is convenient. COPAY'S AND INSURANCE: Please come prepared to pay your co-pay or your co-insurance as specified by your insurance company. We need to verify your insurance at each visit, so please bring your insurance card(s) to all appointments. PRESCRIPTION REFILLS: For routine prescription refills, please call your pharmacy first. If you are unable to get your needs met by the pharmacy, call the office; if you get the office voicemail please leave the following information: your name, the name of the medication(s), the pharmacy telephone number, and your telephone number. We are unable to refill any medications for patients not seen within 12 months of the request. Please try to request refills a week in advance of running out of your medication(s). RECORD REQUESTS: We are happy to provide copies of medical records when requested. If you require just your last office visit and recent lab tests, we will provide this free of charge. For a $30 fee we will copy up to 30 pages of records. We charge 50 cents per page above 30. We need four weeks notice to make copies; however, we will try to have the copy available sooner. We also need a signed medical record request form on file. If you require a complete copy of your chart, for legal proceedings we utilize a copy service. Upon request we will give you their telephone number so that you can arrange to have your chart copied. They provide this service for a fee. You will need to make payment arrangements with the copy service to cover this fee. |
